Our ‘Lessons Learned’ system was upgraded during 2004. This system is used to distribute summary incident reports throughout WMC to enable the ‘lessons learned’ to be communicated and, where necessary, implemented at our sites. The system uses Outlook Public Folders to record the actions arising from Lessons Learned Bulletins. The system was modified during 2004 by classifying Lessons Learned Bulletins into 3 groups: Level 3 Bulletins contain specific instructions, approved by WMC's Executive General Managers, for operations to implement. Successful completion of these actions is monitored using our Assurance Reporting process. Level 2 Bulletins require site General Managers to respond to a set of recommended actions and to report on these actions. Level 1 Bulletins are for information only (although General Managers are required to act on them if they are relevant to their operations). |
