Thirty-nine internal audits of selected Major Hazard Standards at sites were planned for 2004. Thirty-six of these were completed. Seven of the Major Hazard Standards were due to be reviewed during 2004. By year end, five had been reviewed by teams of representatives from all sites. The outstanding reviews will be conducted in early 2005. The review process enabled the reviewed standards to be updated to incorporate technical improvements and also to clarify them in areas where ambiguity existed. Additional guidance information to assist consistent implementation has been developed as part of the review process. Our Major Hazard Standard on Isolations was upgraded and broadened into a Permit-to-Work Standard during 2003. All sites commenced implementation of this new standard during 2004. The new standard requires a high degree of consistency in its interpretation and implementation across the company. It incorporates permits for confined space, high voltage work, excavation, radiation and hot work. |
